Preparing for work, school, or medical forms

Know what to ask before bringing paperwork to an appointment.

Reviewed by Nicholas Atudonyang, MD — October 2026.

Ask before you schedule

Tell the office which form or note you need and when it is due. Ask whether the practice can assess that request, what kind of appointment is required, and whether a separate fee applies. Do not assume a form can be completed at a brief visit.

Bring the complete form

Include instructions, deadlines, and relevant existing records. Some requests require measurements, testing, a hands-on examination, or an assessment by a different clinician. Confirm secure methods for sending paperwork rather than emailing private records.

Understand the limits

A clinician can document findings and recommendations supported by an appropriate evaluation. Completion, certification of fitness, backdating, or approval of a particular accommodation is decided by the clinician and the requesting organization, not promised in advance. Work with the requesting organization if additional information is needed.

Leave time for the process

Ask about expected review time, how to receive the completed document if appropriate, and what follow-up may be necessary. We cannot promise completion on the day of your visit. For routine scheduling, call (936) 439-6070 or text (936) 297-6665.

Emergency symptoms?

Call 911 or go to the nearest emergency department. Do not wait for a routine appointment, phone call back or email reply.

Questions for your next visit?

Call or text Walk-In Wellness to ask about primary care appointments. Time-sensitive concerns may need a different level of care.

A short record of important diagnoses, allergies, major procedures, and recent tests can help you prepare for an appointment. Include the names of other clinicians involved in your care. Ask the office which records are most useful for the visit.

If the practice provides a secure way to submit images, use good light and include both an overall view and a closer view. Do not send sensitive images by ordinary email or text. A photograph may not replace a direct examination.
